Fifty one hundred of clinical samples were collected from patients suffered from different diseases. These samples included urine (45), blood (37), wound (24), burn (23) and sputum (21).The bacterial collection was done during the period from 10\/10\/2017 to 25\/1\/2018 from different hospitals in Baghdad.<\/span><\/h3>\n

The isolates were identified by culturing on MacConkey agar , Eosin Methylene blue agar (EMB) , Blood agar base and\u00a0 CHROMagar orientation media.Then the isolates were identified by several biochemical tests including (Oxidase, catalase, Indole,Methyl-red,Voges-Proskauer,Citrate utilization,kligler iron and Urease).The final identification of isolates was done by using Vitek-2 system .The results showed that 50 isolates were belonged to Klebsiella pneumonia included 15 from urine,12 from blood,9 from each sputum and wounds and 5 from burns.<\/span><\/h3>\n

The antimicrobial susceptibility test exhibited that Klebsiella pneumoniaeisolates were high resistant for Piperacillin 92% ,Ceftazidime and Cefataxime84% each of them, followed byAzetreonam78% ,Cefepime74%,Trimethoprime-Sulfamethoxazole64%, Tobramycin62%, Ciprofloxacin56%, Gentamicin 50% , Tetracycline48%, Amikacin 40% and the lowest resistance was for Imepenem and Ofloxacin 32% each of them and finally Levofloxacin28%.<\/span><\/h3>\n

The Minimum Inhibitory Concentrations (MICs) showed thatKlebsiella pneumoniaeisolates were high resistant for Ampicillin100%, Nitrofurantoin90%, Cefazolin88%,Ceftriaxone86% and Ceftazidime84% followed by Cefepim80%, Trimethoprime-Sulfamethoxazole 72%,Cefoxitin56%,Piperacillin-Tazobactam54%, Gentamicin 50%, Amikacin44%, Ertapenem42%,Imepenem36%,Ciprofloxacin30% and the lowest resistance was for Levofloxacin22% and Tigecycline8%.<\/span><\/h3>\n

The results showed that 50% of Klebsiella \u00a0pneumonia isolates were positive for Extended Spectrum \u03b2-lactamases (ESBLs).<\/span><\/h3>\n

Molecular detection of amino glycosides resistance genes revealed that Klebsiella pneumonia isolates were harboring aac(3\u2032)-\u0399\u0399, aac(6\u2032)-\u0399b, aph(3\u2032)-V\u0399 , ant(3\u2032\u2032)-\u0399 and armA\u00a0 at percentages 94%,86%,36%,16% and 10% respectively. In contrast, aac(6\u2032)-\u0399I, and rmtB were not found among\u00a0 K. pneumonia isolates.<\/span><\/h3>\n

Molecular detection of quinolone resistance genes revealed that all Klebsiella pneumoniae isolates were harboring gyrA and parC.Moreover,Klebsiella pneumoniaeisolates were possessing oxqA, aac(6\u2032)-\u0399b-cr , qnrS,qnrB and oqxBat percentages 94%,76%,76%,36% and 10% respectively. On the other hand, theqnrA, qnrC and qnrD were not found among Klebsiella pneumonia isolates.<\/span><\/h3>\n

The DNA sequencing of \u00a0aac(3\u2032)-II, aac(6\u2032)-Ib, ant(3\u2032\u2032)-I, aph(3\u2032)-VI, armA, qnrB, qnrS,oqxA, oqxB, gyrA and parC genes revealed that the presence of silent mutations, which didn\u2019t\u00a0 cause any substitution on the protein translation and misses mutation, which effected on the protein translation through the substitutions of amino acids<\/span><\/h3>\n
