The Abstract<\/span><\/h3>\n

\u00a0\u00a0 The study is don on (60) sample of bacteria which is isolated from various clinical samples (urine, wounds, blood, sputum and burns), characterized by gram negative, lactose ferment, mucoid, identify it first of all by its shape and appearance of culture and biochemical testes then confirmed it by vitek-2 system, and the final identification don by Polymerase chain reaction which detect (16-23)sr DNA which is specific for Klebsiella pneumonia as a confirmatory diagnoses<\/span><\/h3>\n

All the isolates undergo the tow sensitivity test , the fist test don by minimum inhibitory concentration (MIC) by vitek-2 system ,and the second test was done by disk diffusion method , the result of tow sensitivity test show that: the present of ESBL by 53.3%, and all isolates is resistance to ampicillin by 100%, Piperacillin\/ Tazobactam by 45%, Cefazolin by 85%,\u00a0 Cefoxitin by 46%, Cefotazidim by 80%, Ceftriaxone by 81%,\u00a0 Cefipim by 76.6%, Ertapenem 35%, Imipenem by 30%,\u00a0 Amikacin by 35%, Ciprofloxacin by 26%,\u00a0 Levofloxacin by 18.3%, Gentamicin by 45%, Tigecycline by 5%,Nitrofurantoin by 93.3%, Trimethoprim\/Sulfamethoxazole by 66.6%, Azithromycin by 85.3%, Chloramphenicol by 23.3%.<\/span><\/h3>\n

Cartwheel method was done to detect the efflux pump activity by phenotype, by using red safe as an indicator for the activity of the efflux pumps, after examined and check under UV light, in the isolated that chosen for the study, we found that all of them contain AcrA gen in the chromosome by using PCR<\/span><\/h3>\n

We study the expression of efflux pump gen AcrA by q(RT-PCR) than compare it with 23s housekeeping gen for the sensitive and the resistant \u00a0isolates, before and after exposure to antibiotics (Imipenem, Ciprofloxacin, Amikacin, Chloramphenicol, Azithromycin)<\/span><\/h3>\n

We find over expression of AcrAB efflux pump when exposed to Imipenem, Chloramphenicol and Azithromycin in both isolates (the sensitive and the resistant \u00a0isolates), that mean it could be part or Maine pathway of resistant in resistant isolate, \u00a0increase expression of AcrAB efflux pump when exposed to Ciprofloxacin and Amikacin in sensitive isolate, while decrease expression of AcrAB efflux pump with the same antibiotic in resistant isolate , which is mean the AcrAB efflux pump it\u2019s not a part of resistance in resistant isolate<\/span><\/h3>\n
