      Aim of the study<\/span><\/h3>\n

      The aim of the study was to known the causes of fatty liver disease resulting from hypothyroidism, and whether these is a biomarker associated with this condition. We are focusing on a newly defined peptide hormones, the first was Adropin which has important role in metabolic homeostasis such as controlling on fatty acid metabolism and the second Afamin which is a member of the human albumin gene family and mainly expressed in the liver with the impact study of vascular endothelial growth factor (VEGF) and its receptors which are important players of normal and pathologic angiogenesis. The study was conducted in hypothyroidism patients with and without fatty liver disease and to results with control group.<\/span><\/h3>\n

      The relationship of Adropin, Afamin and VEGF-R2 with anthropometric and clinical features (age, gender, BMI and duration of hypothyroidism) , serum thyroid hormones(T3, T4 and TSH, lipid profile (s.Cholesterol, TG and HDL), liver Enzymes (GOT , GPT and ALP ) levels , kidney function tests (B.Urea and serum Creatinine), glycemic parameters (F.B.S and HbA1c levels), serum hematological parameters(PCV, MCV and WBC) and serum protein\u00a0 parameters (T.protein, s.Albumin and s.Ferritin) levels were also studied.<\/span><\/h3>\n

      Summary<\/span><\/h3>\n

      The study aim is the determination of Adropin, Afamin and VEGF-R2 levels in patients with\u00a0 hypothyroidism and patients with\u00a0 hypothyroidism complicated by fatty liver disease and to compare the results with control group.<\/span><\/h3>\n

      The other aim is to study the relationship of Adropin, Afamin and VEGF-R2 with anthropometric and clinical features (age, gender, BMI and duration of hypothyroidism) , serum thyroid hormones (T3, T4 and TSH, lipid profile (serum cholesterol, TG and HDL), liver enzymes (GOT , GPT and ALP )levels, renal function tests (blood urea and serum creatinine), glycemic parameters (F.B.S and HbA1c levels), Serum Hematological Parameters(PCV, MCV and WBC) and serum protein\u00a0 parameters (total protein, albumin and ferritin) levels.<\/span><\/h3>\n

      Ninety individual participated in this study and classified into three groups:<\/span><\/h3>\n

      G1: Hypothyroidism with fatty liver disease patients, G2: Hypothyroidism without fatty liver disease patients and G3: control subjects.<\/span><\/h3>\n

      All study cases were chosen from Specialized Center for Endocrinology and Diabetes in Baghdad. The age ranges of G1, G2 and G3 were (32-64), (23-67) and (24-40) years, respectively. Gender distribution shows more females than males in all the studied groups.<\/span><\/h3>\n

      Results demonstrated the mean values of serum Adropin in G1 (469.113\u00b139.20pg\/ml) and G2 (561.037\u00b147.6) were significantly decreased (P=0.0001) compared with the mean serum Adropin in G3(2919.73\u00b131.8pg\/ml).<\/span><\/h3>\n

      Moreover, the mean values of serum Afamin in G1 (277.688\u00b115.77 ng\/ml) and G2 (337.020\u00b123 10. ng\/ml) were significantly increased (P=0.0001) compared with the mean serum Afamin in G3 (4.030.73\u00b12.796 ng\/ml.<\/span><\/h3>\n

      Additionally, results demonstrated that the mean values of serum VEGF-R2 in G1 (471.340\u00b1126.522 pg\/ml)and G2(438.113\u00b178.962 pg\/ml) were significantly increased (P=0.0001) compared with the mean serum VEGF-R2 values in G3 (275.590\u00b171.220 pg\/ml.<\/span><\/h3>\n

      Results revealed a non significant decrease in T3 and T4 levels in G1 and G2 compared to G3 and in the G1 compared to G2, while results showed a significant elevation (p<= 0.0001) in TSH levels in G1 and G2 compared to G3 and in G2 compared to G3.<\/span><\/h3>\n

      In addition, results showed a significant elevation in levels of (s. Cholesterol and TG) in G1 and G2 compared to G3) and in G1 compared to G2. While a significant decrease was found in HDL levels in G1 and G2 compared to G3, also a reduction was found in HDL level in G1 compared to G2.<\/span><\/h3>\n

      Results revealed a non-significant difference in GOT levels among the the three studied groups. While, a significant increase in the mean of GPT level in G1 compared to G2 and G3 (p=0.006) was obtained. Additionally, a highly significant elevation in serum ALP levels was noticed in G1 and G2 compared to G3 (p= 0.0001) .<\/span><\/h3>\n

      Results showed a significant decrease in B. Urea and S. Creatinine levels in G1 compared to G2 and G3, while an increased in B. Urea and\u00a0 S. Creatinine levels was noticed in the G2 compared to G3.<\/span><\/h3>\n

      Results revealed a significant elevation in F.B.G and HbA1c levels in G1 compared to G2 and G3, and a significant increase in F.B.G and HbA1c levels in G2 comparing to G3.<\/span><\/h3>\n

      Results showed a non significant reduction in Hemoglobin levels in G1 and G2 compared to G3 and in the G1 compared to G2 (p> 0.05). However, there is a significant decrease in MCV levels in G1 compared to G2 and G3, also in G2 in comparison to G3 (p=0.006).<\/span><\/h3>\n

      Additionally, results illustrated a highly significant elevation in WBC counts in G1 in comparison to G2 and G3 (p=0.0001), and an increase in WBC counts in G2 compared to G3.<\/span><\/h3>\n

      Results revealed a no significant decrease in serum protein parameters (total protein, s. Albumin and s. Ferritin in G1 and G2 in comparison to G3 and in the G1 compared to G2 (p>0.05).<\/span><\/h3>\n

      Serum Adropin values were significantly positively correlated with (BMI and TSH) for G1 patients. On the other hand, serum Adropin values were significantly negatively correlated with afamin in G1 patients. While serum Adropin values were significantly negatively correlated with duration of hypothyroidism and blood Urea for G1 , with fasting blood sugar in G2.<\/span><\/h3>\n

      Serum Afamin values were positively significantly correlated with (T4 and T. protein) for G1. While, serum Afamin values were negatively significantly correlated with TSH and adropin for G1 patients. Moreover, serum Afamin values were significantly positively correlated with the duration of hypothyroidism in G1.<\/span><\/h3>\n

      Serum VEGF-R2 values were\u00a0 significantly positively correlated with F.B.G and HDL in G1.<\/span><\/h3>\n

      Serum VEGF-R2 values were significantly positive correlated with ALP in G1. Similarly serum VEGF-R2 values were significantly negative correlated with Duration of hypothyroidism, S. Creatinine, GOT and GPT for G1.<\/span><\/h3>\n

      The Receiving Operating Characteristic (ROC) curves analysis for serum Adropin, Afamin and VEGF-R2 levels, when used as a test to diagnose subjects as hypothyroidism with and without fatty liver cases (G1 and G2) and control groups(G3) showed that the area under the curve (AUC) of serum Adropin (pg\/ml) and Afamin (ng\/ml) was (1.000) with confidence interval (95% CI) and lower bound (1.000) and upper bound (1.000), while the AUC of serum VEGF-R2(pg\/ml) was (0.918) with confidence interval (95% CI) and lower bound (0.852) and upper bound (0.983).<\/span><\/h3>\n

      Recommendations:<\/span><\/h3>\n

      \u00a0According to the results presented in the current study we can suggest:<\/span><\/h3>\n

      Further researches are needed to elucidate the role of other regulatory peptides in development of metabolic syndrome.<\/span><\/h3>\n

      Implementations of preventive measures to reduce the burden of obesity and hence\u00a0 metabolic syndrome is needed.<\/span><\/h3>\n

      \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Lifestyle changes, weight reduction through dietary modification and regular physical activity improves metabolism. Raising health awareness, therefore, in the society about healthy nutrition and renouncing sedentary life style is necessary.<\/span><\/h3>\n
