The Relationship of Fetuin A level and other Biochemical parameters in type 2 diabetes patient with and without Ischemic Heart Disease<\/span><\/h3>\n

ABSTRACT<\/span><\/h3>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Diabetes mellitus (DM) is a metabolic syndrom characterized by hyperglycaemia because of defective insulin action, insulin secretion impairment or both reasons. Ischemic heart disease (IHD) is one of the complications of diabetes, which is considered a major cause of morbidity & mortality. Insulin\u00a0 and fetuin-A\u00a0 are hormones which play roles in glucose metabolism regulation. Insulin is responsible for regulation of glucose passage to the cells, while fetuin-A blocks insulin binding to its receptor which linked to insulin resistance in metabolic syndrome. In case of higher fetuin-A levels, patients have four times greater risk to develop IHD compared to patients with lower fetuin-A levels. Studies demonstrated that both hormones contribute to type 2 Diabetes (T2DM) and IHD development, in addition to their roles in the pathophysiology of metabolic syndrome.<\/span><\/h3>\n

\u00a0The study also took another hormone called leptin which is associated with insulin resistance and T2DM development. In T2DM, a link between high leptin levels and highly CV risk, leptin is predominantly secreted by adipose tissue as well as other tissues, including heart, through autocrine or paracrine Impacts. Appetite is regulated by leptin through controlling satiety signals to central nervous system and leptin also influences cardiovascular function either directly or indirectly through secondary responses mediated by the vasculature (e.g. hypertension, atherosclerosis, endothelial function and thrombopoiesis) or the CNS.<\/span><\/h3>\n

\u00a0 The current case-control study involved 120 patients (60 T2DM with IHD and 60 T2DM without IHD) their ages ranged between (30-65) years who attended Al-Yarmook teaching hospital and the Iraqi center of<\/span><\/h3>\n

<\/h3>\n

Myocardial \u00a0Infarction, Baghdad teaching hospital \/ Medical city. The study also included 60 healthy individuals with the same age ranges as a control group for comparison. The control group was selected as healthy and not suffering from heart diseases, diabetes, or hypertension. The diagnosis of IHD was based on the medical reports, laboratory and clinical tests for heart disease. Patients with neuropathy and thyroid dysfunction were excluded from the study.<\/span><\/h3>\n

\u00a0For all subjects included in the study both age and BMI are measured by mathematical ways, FBS ,lipid profile (TC,TG,lDL_C,HDL_C,VLDL_C) ,liver function(GPT,GOT),kidney function(urea,creatinine,uricacid)aremeasuredbyspectrophotometer.Fetuin ,leptin\u00a0 ,insulin and HsCRP are measured by ELISA.HbA1C measured\u00a0 by HPLC .<\/span><\/h3>\n

All these factors have major causative roles in the pathogenesis of cardiovascular diseases in these patients. \u00a0<\/span><\/h3>\n

<\/h3>\n

The main finding\u00a0 shown significantly increased in T2DM patient (with IHD) in Fetuin A ,FSG , insulin, HbA1C .HOMA-IR,TC , TG ,LDL-C , VLDL-C, leptin ,urea ,creatinine , uric acid and HsCRP compared to those without IHD and control.<\/span><\/h3>\n

\u00a0 there was\u00a0 significantly increased \u00a0of HDL-C level in T2DM without IHD compared to those with IHD and control , while there was significantly decreased in\u00a0 GOT level in T2DM patient (with IHD) compared to those without IHD and control.<\/span><\/h3>\n
