Also, to estimate the serum M-CSF and POSTN levels in Iraqi osteopenia patients with and without Type 2 diabetes mellitus, as well as determine the levels of vitamin D, calcium, and phosphorous in osteoporosis and osteopenia patients with and without Type 2 diabetes mellitus. And to find the relationship between macrophage colony stimulating factor and periostin with others study parameters in all patient\u2019s groups. One hundred thirty-eight participants were registered in this research study from Baghdad Teaching Hospital in Iraq between October 2021 to March 2022 in the bone density unit. The confirmation of patients (osteoporosis and osteopenia) by Dual-energy x-ray Absorptiometry (DEXA). Subjects were divided into six groups. First group was twenty-seven subjects Osteoporosis patients (7 males and 20 females) without T2DM(Poro).Second group was twenty subjects Osteoporosis patients (8 males and 12 females) with T2DM(Dporo).Third group was twenty-six subjects Osteopenia patients (13 males and 13 females) without T2DM(Peni).Fourth group was twenty subjects Osteopenia patients (5 males and 15 females) with T2DM(Dpeni). Fifth group was twenty subjects T2DM(10 males and 10 females) without Osteoporosis and Osteopenia(D).Sixth group was twenty-five subjects (13 males and 12 females) healthy control group(C), with age range (40-65) years for all patients and control groups had been underwent in this study. The enzyme-linked immune sorbent assay (ELISA) technique was used to estimate the macrophage colony-stimulating factor and periostin levels, while calcium, phosphorous, vitamin D3, HbA1C and FBG level is assessed by biochemistry techniques. The results revealed a significant difference association (p\u2264 0.05) between the six study groups in T-score and Bone Mineral Density (BMD). The results of the study showed a significant increase (p \u2264 0.05) in macrophage colony stimulating factor in the serum of patients have osteoporosis and osteopenia(with\/without diabetes) comparing to healthy subjects. The study also showed a significant increase in periostin level (p \u22640.05) was observed in the serum of patients of all groups compared to healthy subjects. In addition, there was a significant increase (p \u22640.05) in calcium level, while the vitamin D3 and phosphorus levels decreased significantly (p \u22640.05) in all patients groups .According to Receiver Operating Characteristic (ROC) Curve the result of a cut-off value of serum M-CSF was 467.01 ng\/ml as a predictor of osteoporosis diagnosis and levels 523.22ng\/ml represent an osteoporosis with T2DM, as well as sensitivity and 1- specificity was 100%. The interpretation of periostin ROC curve results also demonstrated the cut-off value of osteoporosis was more than 1.02 ng\/ml, and osteoporosis with T2DM was 0.962 ng\/ml.<\/span><\/h3>\n<h3 style=\"text-align: left;\"><span style=\"color: #993366;\">In conclusion: This study is the first to examine the association between serum macrophage colony stimulating factor and type 2 diabetes mellitus in Iraqi patients with osteoporosis. Even with clinically normal reference values of Ca and phosphate, but this study confirms the hypothesis that osteoporosis produces a decrease in total mineralized bone.
